Milrinone Lactate
Milrinone lactate is a bipyridine phosphodiesterase 3 (PDE3) inhibitor with positive inotropic and vasodilator activity. It works by increasing intracellular cyclic adenosine monophosphate (cAMP) levels in cardiac and vascular muscle, resulting in enhanced myocardial contractility and relaxed vascular smooth muscle, which reduces both preload and afterload.
Dosage by Body Weight
Administered strictly intravenously. The standard adult regimen includes a loading dose of 50 mcg/kg administered slowly over 10 minutes, followed by a continuous maintenance infusion ranging from 0.375 mcg/kg/min to 0.75 mcg/kg/min, titrated based on hemodynamic clinical response and renal function.
- Body Weight
- Below 5 kg
- Dose
- 0.375 to 0.75 mcg/kg/min
- Frequency
- Continuous IV infusion
- Tracking Notes
- Safety and efficacy in pediatric patients are not fully established. Administer only in a neonatal or pediatric ICU setting under intensive monitoring.
- Body Weight
- 5-7 kg
- Dose
- 1.875 to 5.25 mcg/min (calculated from 0.375 - 0.75 mcg/kg/min)
- Frequency
- Continuous IV infusion
- Tracking Notes
- Requires continuous ECG and blood pressure monitoring. Reduce dose if renal impairment is present.
- Body Weight
- 8-10 kg
- Dose
- 3.0 to 7.5 mcg/min (calculated from 0.375 - 0.75 mcg/kg/min)
- Frequency
- Continuous IV infusion
- Tracking Notes
- Monitor fluid and electrolyte status, especially potassium levels, as hypokalemia may predispose to arrhythmias.
- Body Weight
- 11-15 kg
- Dose
- 4.125 to 11.25 mcg/min (calculated from 0.375 - 0.75 mcg/kg/min)
- Frequency
- Continuous IV infusion
- Tracking Notes
- Avoid in patients with severe obstructive aortic or pulmonic valvular disease.
- Body Weight
- 16-20 kg
- Dose
- 6.0 to 15.0 mcg/min (calculated from 0.375 - 0.75 mcg/kg/min)
- Frequency
- Continuous IV infusion
- Tracking Notes
- Infusion rate should be carefully titrated according to hemodynamic parameters and clinical response.
